The first appointment is an opportunity for one of our providers to get to know you and your family. We typically ask a lot of questions to best understand your concerns and reasons for seeking treatment.
The following information is intended to help you better understand what to expect and address questions that you may have about your first appointment:

Please plan on arriving 10 minutes early for this appointment to complete initial paperwork that include HIPPA Policies, Informed Consent and Patient Agreement.

We will obtain a copy of the insurance card and driver’s license from the individual responsible for insurance coverage.

Please complete the Initial Questionnaire that was sent via email and can also be downloaded here.

Feel free to bring in any other documents including: report cards, IEP or 504 Plan, previous psychological or educational testing, and any other information you feel would be helpful.

Children under 18 must be accompanied by a parent or guardian. However, you may attend the initial meeting without your child present.

Upon arriving at 800 Main Street, please drive around the building to find plenty of parking and our private entrance.
